$30,000 to the Housing Authority for the addition of a Senior Assessment
Coordinator for seniors living in public housing.
$20,000 to LOGAN Center for the rehabilitation of their facility
which houses their senior day program.
$47,292 to LOGAN for the first phase of a two-phase
project of enhancements to the facility-bathroom and personal care
area, and the addition of two exits and a walkway. The second phase
will be funded through private contributions. These project enhancements
will ensure that LOGAN can continue to answer the needs of seniors
with development disabilities by providing them with a specialized
day program that fosters an essential sense of independence and promotes
integration into the community.
$333,334 to REAL Services, Inc. for the Aging in
Place Home Modification Collaborative. This grant will assist
the elderly to remain in their homes by providing needed home modifications. The
project targets the homes of low-income elderly residents of South
Bend. REAL Services will provide community outreach, identify
potential program participants, screen homeowners, contract with
companies to provide home modifications, and monitor and pay contractors. This
funding will match money from two other funders which will total
$1 Million. The objective is to provide home modifications
for 100 seniors over the course of a year.
